I was kind of surprised nobody had commented on this one either. The dream was inspired by the "Fall on your knees" prompt. The second section was taken from the "A New Man" (http://en.wikipedia.org/wiki/A_New_Man) episode from season 4. I watched the episode and pretty much wrote down what happened althought the thoughts in Ethan's head are mine.
The last bit was all me although the "what happens if Ripper disappoints" happens in the rest of the episode. The link above gives a synopsis if you're not familiar with it.
