Yup, from a prank devised by Sirius gone too far that could've costed Snape his life, or turned him into a werewolf. Some metas make the claim that in the world of magic, debts, especially those where you owe someone your life, must be repaid. Snape would eventually do so by sacrifising his own life to protect Harry Potter.

I recall from a fanfic that Snape had been exposed, by Sirius, to Lupin's werewolf form as a prank. Is that it?

I prefer the idea that Snape sacrificed himself because of his love for Lily.
